"All reasonable measures to protect the confidentiality of your records and your identity will be taken."  This is not treatment study. While some laws require that participants are not discriminated against for participating in a research study, you may wish to discuss this matter with your insurer.  Protected Health Information (PHI) is health information that identifies you. PHI is protected by federal law under HIPAA (the Health Insurance Portability and Accountability Act). To take part in this research, you must give the research team permission to use and disclose (share) your PHI for the study (as will be explained in a consent form). We will ask you questions about your health and record this information in your research file. However, the research staff of this study maintains the confidentiality of your records by keeping your interviews, consent forms, identification codes, and other contact forms in locked file cabinets. Your interview will contain only code numbers rather than your name. All forms containing information identifying you, your friends or relatives will be kept separate from your interview. Your identity will not be revealed in any publication that may result from this study.  In addition to health information that may be created by the study, the research team may access the following sources of your health information to conduct the study:

  • Information to find and contact you for potential follow-up assessments
  • Vital statistic information that may contain your health status information
The research team may share your information with:
  • Government representatives (such as the Food and Drug Administration or the Office of Human Research Protections) to complete federal or state responsibilities
  • Hospital or University representatives, to complete Hospital or University responsibilities
  • Your primary care physician if a medical condition that needs urgent attention is discover December 28, 2005 on the General Clinical Research Center (GCRC), if GCRC facility is used to conduct any portion of the study
  • Mental health professionals and appropriate authorities/services if the interviewer determines that you are a danger to yourself or others.
